BEFORE SENDING YOUR SAMPLES
Fill out this form so that we have some notice that you’re sending samples. Please note that you DO still need to print, fill out, and submit a paper copy of the order form when you send your samples in. You will find several resources below including a slide show and the order form.
VERY IMPORTANT!! We only accept samples taken with Scotch brand Transparent tape, regular size, NOT PACKING TAPE or any other brand. You can get it at Walmart or Walgreens.
